If you are a new hire, you have 60 days to enroll in Life up to the guaranteed issue limit  (Employee, $100,000 or Spouse $50,000) without the need to answer medical questions.

The Optional Life Insurance coverage is a voluntary benefit in which you pay the entire premium.

Optional Life coverage is available in $10,000 increments up to the lessor of 6x salary or $750,000.

Your Optional Life coverage cost is based on the amount of coverage you elect and your age as of April 1, 2024. These rates are guaranteed until April 1, 2025.

The Dependent Spouse Life Insurance coverage is a voluntary benefit in which you pay the entire premium. 

Dependent Spouse Life coverage is available in $10,000 increments up to $150,000 or 100% of your Optional Life amount, whichever is less. 

Your Dependent Spouse Life coverage cost is based on the amount of coverage you elect and your age as of April 1, 2024. These rates are guaranteed until April 1, 2025.

The Dependent Child Life Insurance coverage is a voluntary benefit in which you pay the entire premium.

Dependent Child Life coverage is available for $5,000 or $10,000. Children are eligible from live birth to age 26. A child can be covered by only one parent.

One monthly premium covers all eligible children. Your Dependent Child Life coverage cost is based on the amount of coverage you elect as of April 1, 2024. These rates are guaranteed until April 1, 2025.
