How to Appeal Against a Claim that denied
If you’re unhappy with the outcome of a processed claim, you can ask for the claim to be reconsidered for payment. It’s suggested you work with your dentist to appeal a claim.
The information required for a reconsideration of a denied claim will be:
- a brief letter of explanation
- a copy of the original Explanation of Benefits statement
- any new or additional information (charting, x-rays, narrative notes)
MetLife will reconsider any dental claim, but new or additional information from your dentist is needed to properly evaluate the request. Your dentist should submit the request for reconsideration to the address listed on the Explanation of Benefits (EOB) statement.
