MetLife’s Long-Term Care Guide to Making a Change in Provider or Care.

**Please note: Service providers are approved based on your policy terms and provisions

Contact us at 888.687.0977 before you change your care or add a new provider, so that we can verify the provider is approved in accordance with your policy criteria. This call will ensure that MetLife is aware of the change in care and your new provider is approved, to prevent delays in your claim process.

Information required for Change of Provider or Care includes:

  • Facility – (Nursing Home, Assisted Living or Inpatient Hospice) - Name, address, contact number, license number, and tax ID number of the facility. Start of service date if applicable.
  • Home Care – (Home Care Agency, Adult Day Care or Home Care Hospice) -Name, address, contact number, license number, and tax ID number of agency. If applicable, start of service date, rate of pay and frequency of service.
  • Informal Care- Name and phone number of the new caregiver. Send a copy of state issued ID Card or driver’s license card to our long-term care department via fax, email or mail. If applicable, start of service date, rate of pay and frequency of service.
  • Independent Care Providers (IP)CNA, HHA, LPN, LVN, RN and Therapist)- Name and phone number of the new IP. Send a copy of state issued ID Card, or driver’s license card along with copy of credential to our long-term care department, via fax, email or mail. If applicable, start of service date, rate of pay and frequency of service.

Once your new provider or caregiver is approved, we will require invoices or monthly timesheets for reimbursement.
