For life insurance new hire elections and open enrollment changes that require medical evidence, employees and employers use an electronic process. Employers should encourage employees to submit all open enrollment changes through MyBenefits except for life insurance elections that require medical evidence. Do not hold other open enrollment changes while waiting for approval of life insurance elections. Below is a summary of the electronic process, but more details are available online.

 

  • The employee must complete an Active Notice of Election (NOE), sign it and return it to their benefits administrator. List the total coverage amount in Box 24 for Optional Life coverage or Box 23 for Dependent Life-Spouse.
  • The employer will use the information on the NOE to submit a Life Insurance Statement of Health Request in EBS by selecting the Life Ins SOH button on the homepage.
  • Each Friday, PEBA sends a file to MetLife that includes any requests submitted in EBS during the week. MetLife uses the information on the file to email the Statement of Health link to employees through the MetLife MyBenefits platform.
    • Employers cannot submit medical evidence requests in EBS for spouses if the employee does not have coverage. Please provide these employees with your custom Statement of Health form if medical evidence is required.
  • If a spouse needs to provide medical evidence, the employee will receive the email from MetLife with instructions to provide their spouse’s email address. Due to HIPAA, the spouse must register their own MetLife MyBenefits account to submit their Statement of Health.
  • Upon completion of the Statement of Health, the employee and/or spouse will receive real-time notification of their status.
  • When an employer receives an approval from MetLife via the weekly report, they will submit the completed NOE and copy of the report to PEBA.
  • The weekly Statement of Health reports are generated each Monday morning.
  • New benefits administrators for an employer who already participates in the insurance benefits administered by PEBA can email SCPEBA@metlife.com to request to receive the weekly reports.
  • When a new employer joins the state insurance benefits program, MetLife adds the benefits administrator to the distribution list to receive weekly reports. The benefits administrator will receive a system generated email to which they need to reply to complete the setup and begin receiving reports.
