Need to Take Leave? Follow These Steps.

If you’ve experienced an injury, are tending to an illness, find yourself caring for a family member (including a newborn), military exigency or military family care, you might need to step back from work for a period of time. Here’s how to file a claim properly, so you can take the time you need—while receiving financial support.

Here’s what you need to know about taking leave

Whether you require Short-Term support through the Family and Medical Leave Act (FMLA), and California Family Rights Act (CFRA), Pregnancy Disability Leave (PDL) please notify your manager and file your claim at least 30 days before the first date you intend to take off. If your leave request is the result of a sudden occurrence, file your claim as soon as possible. When taking intermittent time, you will need to report your absence within two days.

Before taking leave, you’ll need to provide MetLife with supporting medical documentation for you or your family member, except where prohibited by law. MetLife’s MyBenefits portal makes it easy to submit a claim, upload necessary documentation, and track the claim’s status. Ready to return to work? Let your manager know your anticipated end date and inform MetLife when you’re back so they can close your claim.

Employee Action Items

  • Inform your manager/supervisor, at least 30 days in advance when possible
  • File your claim online by registering on mybenefits.metlife.com. Updates and helpful resources are also available on the site.
  • Or contact MetLife at 855 229-7305
  • When received, please sign the MetLife medical authorization. This will allow MetLife to get the information they need from your doctor on your behalf.
  • When returning to work, please notify the manager/supervisor and MetLife.

Return to Work Checklist

  • Please discuss your return to work with your manager/supervisor. You must comply with any San Bernardino County policies regarding your return to work.
  • Please report your return to work to MetLife as soon as you have returned to work.
  • If you have restrictions and/or limitations that prevent you from returning to work, and you require accommodations to return, please discuss these accommodations with your manager/supervisor and MetLife claims specialist.
