If you need to report a claim for an event that occurred on or before 12/31/2023, you may currently file a claim through MetLife’s MyBenefits portal (the portal is only available until December 31, 2024) or call (1-877-770-4638, prompt 3) to request a claim form by phone. Beginning 1/1/2025, online claim filing will no longer be available, and all claims will need to be initiated by calling MetLife.

MetLife will no longer be Intel’s Critical Illness carrier beginning 1/1/2024. If you need to report a claim for an event that occurred on or after 1/1/2024, you need to contact Intel’s new critical illness carrier in order to file the claim. If you have questions on how to submit a claim with the new carrier please contact Intel’s AskHR support at 800-238-0486.
