Need to Take Leave? Follow These Steps.

If you’ve experienced an injury, are tending to an illness, find yourself caring for a family member (including a newborn), military exigency or military family care, you might need to step back from work for a period of time. Here’s how to file a claim properly, so you can take the time you need—while receiving financial support.

Getting Started

Here’s what you need to know about taking leave

Whether you require Short-Term or Long-Term Disability benefits, or seek support through the Family and Medical Leave Act (FMLA), Massachusetts Paid Family and Medical Leave (MA PFML), Massachusetts Parental Leave (MA PL),  New York Disability Benefit Law (NY DBL) and Paid Family Leave (NY PFL) or New Hampshire Paid Family and Medical Leave (NH PFML) please notify your manager and file your claim at least 30 days before the first date you intend to take off. If your leave request is the result of a sudden occurrence, file your claim as soon as possible.

Staff Member Action Items

  • Inform your manager, at least 30 days in advance when possible
  • Contact MetLife at 833-431-1439
  • Register on mybenefits.metlife.com for updates and helpful resources. When registering on MyBenefits you will need a 9-digit ID # or social security number. If using your employee ID, please add the appropriate number of zero’s in front of your ID # to total 9 digits (omit any letters).
  • When received, please sign the MetLife medical authorization
  • Complete time entry form to supplement leave benefits with available Dana-Farber accrued time
  • Coordinate your return to work between your doctors and Dana-Farber

Return to Work Checklist

  • Please discuss your return to work with your supervisor. You must comply with any Dana-Farber policies regarding your return to work.
  • Please report your return to work to MetLife as soon as you have returned to work.
  • If you have restrictions and/or limitations that prevent you from returning to work, and you require accommodations to return, please discuss these accommodations with your claims specialist.
