Providers enrolled in the CCPU Health Care Fund Reimbursement Program are automatically enrolled in MetLife dental and vision coverage at no cost.*
Childcare providers who are active and enrolled in the medical CCPU Healthcare Reimbursement plan are automatically enrolled into a dental and vision plan. There is no action needed by childcare providers.
You can go to any licensed eye care professional.6 Choose from the thousands of ophthalmologists, optometrists and opticians at private practices or popular retail locations like Costco Optical, America’s Best, Cohen’s Fashion Optical, Eyeglass World, For Eyes Optical, Pearle Vision,7 Sam’s Club, Shopko, Visionworks, Walmart and more.8
You can choose the eyewear that’s right for you and your budget. Your eye care professional can help you choose from classic styles to the latest designer frames. You can select from hundreds of options for you and your family. Some of the great brands to choose from include Anne Klein, bebe, Flexon, Lacoste, Nike, Nine West, Calvin Klein, and more.8
