Report a Death
(an annuitant is a person who receives regular payments from a pension or an annuity investment.)
We are sorry for your loss.
This process is used to report the death of a retiree or survivor annuitant who is currently receiving a benefit from OPM under the Civil Service Retirement System (CSRS) or the Federal Employees Retirement System (FERS).
You can report the death in one of three ways
Carefully complete the "Report Annuitant Death" form located on the OPM website : Click here
Phone: Call OPM Retirement Services at 1-888-767-6738 (1-88USOPMRET) from 7:50 AM to 5:00 PM (UTC). The phone lines may be busy, so try calling early in the morning or after 4:00 PM.
Mail: Write a letter to: U.S. Office of Personnel Management, Retirement Operations Center, P.O. Box 45, Boyers, PA 16017-0045. Include the annuitant's full name, date of death, retirement claim number and/or social security number.

- Please contact the human resources office of employee's employing agency. You will need to provide the employee's full name, social security number, and date of death.
