Filing a Death Claim

A beneficiary is a person or entity that is designated to receive the life insurance benefits in the event of a death. One or more beneficiaries can be named and insured can change them at any time. The insured can also specify how the benefits will be divided among your beneficiaries or leave that decision to OFEGLI.

We are sorry for your loss. To claim these benefits, you need to:

1. Carefully complete the "Claim for Death Benefits" form FE-6 that is located on the OPM website (Click here).

2. Obtain a certified copy of the death certificate. This is a copy with an official stamp or signature.

Claiming FEGLI benefits of an Eligible Dependent

We are sorry for your loss. If you are enrolled in Option C Family Life Insurance Coverage and have an eligible dependent under Federal Employees' Group Life Insurance (FEGLI), you may get a lump-sum payment and a change in your annuity reduction. Carefully complete the " FE-6 DEP, Statement of Claim, Option C -- Family Life Insurance " form that is located on the OPM website (Click here). If you need help, contact OFEGLI at 1-800-633-4542.

If you are an employee, send the completed form, with a certified copy of the death certificate, to your employing agency.

If you are an annuitant, send the completed form, with a certified copy of the death certificate, to:
